Documentation of School Health
The Documentation of School Health web-based documentation and reporting system is a tool available to school health team members or health champions. This system allows the compilation of data and will demonstrate an improved overall healthy school culture as it relates to health education. This online documentation tool also allows school advocates to have access to critical outcome based data, which will assist them to reach the health related objectives they have identified.

Our Mission: Henry Ford Macomb School Health Network’s mission is to create a healthier school environment for all students in Macomb County. We are an advocate for the school community to educate, inspire and support healthy lifestyle behaviors, which encompass body, mind and spirit.
We are achieving our mission by addressing health related issues at the school building level, raising awareness of health promotion opportunities and providing resources to the school community which will allow schools to improve and maintain a healthy environment.
How the program works:
The HFM School Health Network provides support to the school communities in a variety of ways including:
- Development and support of Coordinated School Health Teams
- Mentoring of school health teams and their leaders by well qualified School Health Coordinators.
- Staff development and continuing education opportunities
- Provision of a free Resource Center providing books, articles, videos and health education tool kits
- Facilitation of the Michigan Healthy School Action Tool (HSAT) and the Healthy School Report Card
- A comprehensive program that addresses the concerns of the overweight and their family
- Resource directory of availabe agencies that provide presentations and services to schools for health promotion and disease prevention
- Distribution of a quartery newsletter that updates health team members regarding websites, educational opportunities, community events, information on current health care concerns and health team success stories
- Facilitation of clinical opportunities for university and community college health career students.
We have partnerships with many schools in Macomb County. Together we work toward healthy outcomes for our school communities. In 2017, our programs and interventions reached over 50,800 individuals and provided more than 3500 hours of education.
HFM School Health Network is a proven program that demonstrates there is power in numbers. Our collaborative approach with schools and community resources provides a means of sharing best practices and outcomes, along with supporting one another in our endeavors to improve the health of students in our community.
